Good-Grade Pills were only made by the Advanced Alchemist. But even if they could only produce very few Good-Grade Pills, most of them would just be fine-grade.

Great-Grade Pills were only made by Peak Alchemist. And just like an Advanced Alchemist, they could only create a few Great-Grade Pills in one batch.

As for Perfect-Grade Pills, they weren’t usually found.
